Laparoscopy vs. Open Surgery: What’s the Difference and Which Is Better?

16 July, 2025 | By Dr. Deepu Thottath

The field of surgery has changed quite a bit over the years, leading to different ways of operating, leading to improvements in recovery times and patient outcomes. The most prevalent and often debated among surgery techniques is laparoscopy versus open surgery among General Surgery.

Patients are increasingly curious about these two approaches, particularly in terms of recovery, safety, effectiveness, and suitability. But what exactly sets them apart? And more importantly, which one is better for you?

What is the difference between laparoscopy and open surgery in general surgery?

The main difference with laparoscopy versus open surgery is how the surgery is performed. In open surgery, a traditional manner of surgery, the surgeon takes one big incision and can visually see the tissues or organ directly. The surgeon can then have full access to visualize and have sensation of the area and may have longer recovery times, experience more pain, and will have a greater risk of complications including infections or a major scar.

Laparoscopy, on the other hand, involves smaller incisions, through which a laparoscope (a long tube fitted with a camera and light) and specialised surgical tools are inserted. The camera transmits images to a screen, guiding the surgeon through the procedure. This approach is classified as minimally invasive, giving patients quicker recovery, less pain after the surgery and much less scarring.

Is laparoscopic surgery better than open surgery?

Not necessarily. Laparoscopic surgery is preferred because it is less painful and is considered to be less invasive, but there are always exceptions. For certain patients and certain procedures, laparoscopic surgery is not indicated. Laparoscopic surgery can easily be rendered impossible or ill-advised from time to time for patients with complex issues and/or adhesions or other types of internal scar tissue and of procedures in which good tactile sensation plays a critical role. Ultimately, what the best surgical choice is, a lot depends on the condition, body systems involved, and the overall health of the person having the surgery as well as the surgeon's goals for the surgery.
